Neighborhood-Level Health Outcomes and Social Determinants of Health in Colorado

In this analysis, neighborhood-level health outcomes and social determinants of health in Colorado’s urban and rural areas are compared using the interactive Health Equity Analysis tool. The data tool, developed by HSRI and the Center for Improving Value in Health Care (CIVHC), provides information about social determinants of health and health care measures in Colorado. The tool was developed to help policy makers, providers, public health entities, and communities answer questions about access, outcomes, and overall impact of social determinants of health and health care.
