Louisiana Cross Population Quality Improvement
(2006-2009)HSRI, under contract with the State of Louisiana Department of Health and Hospitals (DHH) through a Systems Transformation Grant funded by the Centers for Medicare and Medicaid Services, assisted DHH in developing a comprehensive quality management strategy consistent with the state’s transformation of its long term support system for individuals with disabilities, elders and individuals with developmental disabilities.

Technical Assistance Provider to CMS 2004 Real Choice Systems Change Mental Health Transformation Grantees
(2005-2009)This project provided ongoing technical assistance and training opportunities to 10 states awarded grants in the mental health area. The purpose was to improve the ability of States to offer evidence-based and recovery-oriented services to consumers with mental illnesses with support of the Medicaid system.

Consumer Centered Quality Assurance and Quality Improvement in Home and Community Based Services
(2004-2006)This 2 year 9 month project was funded by the Delaware Division of Developmental Disabilities Services through a CMS Real Choice Systems Change grant. The purpose was to assist the Division and its stakeholders to develop a more person-centered Quality Assurance and Improvement (QA/QI) System that aligned with the CMS Quality Framework and Interim Procedural Guidance.

Development of Performance Measurements for the Georgia Developmental Disabilities System
(2004)The Georgia Department of Human Resources has contracted with HSRI to assist in the development of performance indicators, measuires and data sources for services to people with developmental disabilities. The project is part of Georgia's System Change Grant funded by the Centers for Medicare and Medicaid Services.
